Aqad for Korban

Please read this Korban declaration:
I, [Your Name], hereby appoint NASAR.SG to carry out my Qurban (or a Qurban entrusted to me) by purchasing a goat/cow/camel in Makkah/Yemen/Uganda/Jordan.
I grant permission to NASAR.SG and its supplier to perform the slaughter and to distribute all of the meat to those in need.
I also authorize NASAR.SG and its supplier to make the best decisions necessary to ensure this ibadah is carried out properly and in accordance with Islamic teachings. May Allah accept this sacrifice as a sincere act of worship from me.

Saya (nama anda) dengan ini melantik NASAR.SG untuk menjalankan ibadah Korban saya (atau korban yang diwakilkan kepada saya) dengan membeli seekor kambing/ lembu/ unta di Makkah/Yemen/Uganda/Jordan.
Saya memberi keizinan kepada NASAR.SG dan pembekal mereka untuk melakukan sembelihan dan membahagikan seluruh daging kepada yang memerlukan.
Saya juga memberi kuasa kepada NASAR.SG dan pembekalnya untuk membuat keputusan terbaik demi memastikan ibadah ini dijalankan dengan sempurna dan mengikut ajaran Islam. Semoga Allah menerima korban ini sebagai ibadah yang ikhlas dari saya.
